CRITIQUE THAT CHALAZION

Oh no… you have this red, ugly bump on your eyelid.


Welcome to Critique That Chalazion, my clinical breakdown of what’s really going on and how to treat it the right way.


I’m a UC Berkeley trained optometrist, and today we’re analyzing a chalazion that’s showing some red flags (literally).

My Critique

🚩 1. The Redness

A little redness? Expected.
But when that redness starts spreading across the eyelid, it raises concern.

At that point, I’m not just thinking “chalazion” anymore—I’m thinking:

  • Could this actually be a stye (an infection)?
  • Is there a risk of preseptal cellulitis?

That’s why in some cases, I prescribe oral antibiotics just in case things escalate. This is not something to ignore.


🚫 2. The Urge to Pop It

Let’s be honest:

  • It looks poppable
  • It feels poppable
  • It wants to pop

But here’s the problem…

That yellowish “head” you see? That outpouching?
It’s often pointing in the wrong direction . . . toward the eyelid skin instead of the natural drainage site, the eyelid margin.

If it drains outward:

  • You risk skin damage
  • You delay healing
  • You increase scarring potential

My Golden Rule: CONTROL THE EXIT SITE

The goal is simple, but critical:

👉 Create and keep an exit site open at the eyelid margin (where oil gland openings live).

Not the eyelid skin. Never the eyelid skin.


The At-Home Routine (Using the We Love Eyes MGD Cleansing System)

This is your step-by-step protocol for this type of chalazion.

STEP 1: Gentle Heat + Targeted Massage

Use a heat tool like a Focused Eye Roll under warm (not hot) water.

Why warm, not hot?
This eyelid is already inflamed. Too much heat = more irritation.

How to use:

  • Always test temperature on your wrist
  • Roll side-to-side gently
  • When moving up/down → light pressure only
  • Always guide movement downward toward the eyelid margin

👉 You are encouraging drainage through the correct pathway

STEP 2: Create the Exit Site (This Is Where Most People Mess Up)

This step changes everything.

Using this DUO:

Application technique:

  1. Place ONE drop of oil onto the brush
  2. Shake off excess → you want damp, not dripping
  3. Use a 10x lighted magnifying mirror for accuracy and safety
  4. Lift the eyelid upward
    • Slight inward push helps expose the margin
  5. Scrub directly at:
    • The eyelid margin adjacent to the chalazion
    • Target the eyelid margin (exit site) once a day before bedtime

Important:
Even if you can’t fully “roll” the lid due to the bump, just exposing the eyelid margin is good enough. Please watch the video tutorial so you can visualize these words.

👉 This step:

  • Opens the blocked gland
  • Keeps the drainage pathway clear
  • Prevents the chalazion from pushing outward through eyelid skin now that there is an exit!

STEP 3: Final Wash-Off (Do NOT Skip)

After the ROLL and SCRUB steps, hopefully you’ve created some chalazion drainage debris.

Now you need to wash it away properly while protecting the eyelid skin barrier.

Use Tea Tree Eyelid & Eyelash Foaming Cleanser. It's a probiotic foaming cleanser that will cleanse, balance and calm the eyelid area. If you need something without tea tree oil, use SuperGLA Eyelid & Eyelash Foaming Cleanser instead. 

  • Apply a small amount to fingertips
  • Gently sweep across closed eyelids (1–4 passes)
  • Rinse with water, towel dry

👉 This supports a healthy eyelid skin barrier, which is essential for healing

Full Routine Recap

  1. Focused Eye Roll → Warm, gentle, downward drainage
  2. Eyelid Margin Scrub Brush + Cleansing Oil → Create & maintain exit site
  3. Foaming Cleanser → Remove debris + support skin healing

A Tricky Chalazion

This type of chalazion is tricky because:

  • It looks ready to pop, but in the wrong place (the eyelid skin)
  • But it’s often heading in the wrong direction
  • It's temping to pop
  • The eyelid is really red which can sometimes mean PRESEPTAL CELLULITIS which requires oral antibiotic because it's an infected stye, aka hordeolum.

Your job is to:
✔️ Redirect it
✔️ Control the drainage
✔️ Protect the eyelid skin
